As a service to our patients we will provide an estimate of costs prior to services rendered. All co-payments or other out-of-pocket expenses will be due at time of service. Dental Associates will submit all claims directly to the patient's insurance company. The insurance company will determine the benefit reimbursement. Any remaining balance is the patient's responsibility.

he was pretty rough

before my extraction, he started with numbing. however he jammed the needle in until he was poking something hard then released the numbing agent, this was excruciating. i told him this but he disregarded it. its been a week and the pain is still excruciating and also needed a follow up appointment. i am allowed to take tylenol and ibuprofen but not in large quantities or for long periods of time. after the procedure, he told me to take 600mg of ibuprofen and tylenol with it xtra strength for pain but said i would be good in 2-3 days. i refuse to see him ever again.

Listening to the radio and making small talk during a dental, surgical procedure does not inspire confidence, particularly as repeated attempts have been made to correct a botched dental implant procedure.
