Dental Patient Bill of Rights:

You have a right
To quality dental care provided in a safe and effective manner

You have a right
To expect dental team members to use appropriate infection control and sterilization methods.

You have a right
To know the education and training of your dentist and the dental team.

You have a right
To ask your dentist to explain all treatment options regardless of coverage or cost.

You have a right
To ask about treatment alternatives, including advantages and disadvantages.

You have a right
To schedule an appointment with your dentist in a timely manner.

You have a right
To reasonable arrangements for dental care and emergency treatment.

You have a right
To know in advance the expected cost of your dental treatment.


Dental Patient Responsibilities:

You have the responsibility
To provide, to the best of your ability, accurate, honest and complete information about your medical and dental history and current health status.

You have the responsibility
To understand and sign an informed consent.

You have the responsibility
To understand the necessity and importance of diagnostic procedures recommended by your dentist.

You have the responsibility
To adhere to regular oral health home care recommendations.

You have the responsibility
For consequences resulting from declining treatment or from not following the agreed upon treatment plan.

You have the responsibility
For appropriate behavior allowing for the delivery of quality dental care.

You have the responsibility
For maintaining appointments in a regular and timely manner

You have the responsibility
To assure that your financial obligations for dental care received are fulfilled.
