As a service to our patients we will provide an estimate of costs prior to services rendered. All co-payments or other out-of-pocket expenses will be due at time of service. Dental Associates will submit all claims directly to the patient's insurance company. The insurance company will determine the benefit reimbursement. Any remaining balance is the patient's responsibility.

He was crazy funny
A 1 on the staff. Everyone friendly, cheerful, and accomodating.
He was the only good part about this visit.
He never responded to the issue I was in for even though he was asked several times by the assistant, my wife and myself.
My Dr was awesome.
Very friendly,personable,kept the visit as comfortable and laid back as I can imagine.
Dentist was very nice and quick but didnt explain what he was using to numb the area, how many shots he was going to give, how long the numbness would last, and no instructions on post filing care.
