As a service to our patients we will provide an estimate of costs prior to services rendered. All co-payments or other out-of-pocket expenses will be due at time of service. Dental Associates will submit all claims directly to the patient's insurance company. The insurance company will determine the benefit reimbursement. Any remaining balance is the patient's responsibility.

I had no idea my treatment was with the same dentist that caused all of my troubles with this tooth in the first place. He took no responsibility for putting a crown on a rotten tooth that caused a serious infection, requiring the root canal in a brand new crown. He is always rushed because his schedule is overbooked. I will not be returning to this dentist.
Dr. Lukats is a very good Dentist and he relaxes you as well.

I often feel pain through the anesthetic (commented to dentist & assistant). I asked if warm water could be used during drilling - dentist said cold water is needed to cool the drill. LAter in procedure I winced again & began crying- I repeated my request & he said I should have told him I was in discomfort (I HAD). At that point, he became concerned for my pain tolerance & reapplied anesthetic to the area.
Great experience. It's not easy to put me at ease but he did. He was very thorough.

saw the dentist for maybe two minutes. he quickly looked at my teeth. did not ask about or examine my receding gum. he looked at my teeth & just said looks good.

I have never had a dentist spent as much time going over the x-rays and explaining what was going on. It helped me to know that I will be receiving the correct treatment.

An unfortunate slip of the dental instrument resulted into a tongue injury that is still causing discomfort 5 days later. The dentist did call to check on how I was doing.
The dentist seemed competent and they found and described a problem. Expected a cleaning with this appointment but none was given or offered.

The dentist was absolutely fantastic!!! He talked me through every step of the way, had a great disposition, and was great while explaining procedural issues to the young man assisting him. He spent a great deal of time making sure I would be comfortable with the temporary crown that I was getting.
My dentist took great care to treatment and offered suggestions and honored my requests. I would hope to return to him for future care. He assured I could call him at anytime with any concerns.

dr Lukats was wonderful, I had a tough time returning to see a dentist after a number of years. He was understanding, took time to explain, and when I needed to have an unexpected emergency root canal, he came to see that I was Ok !,,

The dentist could provide general statements to me regarding the overall health of my teeth. He didn't do that and I would have liked to have his general opinion.

This particular dentist wasnt concerned with my issues. Looked at xray and said see Dr. Brums. Dentist could have reviewed my xray, called me back just to tell me to see Dr. Brums. My time was wasted on this particular trip! My first time every seeing or hearing about this particular dentist.
This particular dentist gets a very poor rating from me. I would not recommend him to others, not at this time.

He had a great sense of humor as well!
My gums were sore (inflamed) after I left. The dental hygienist either brushed my teeth to hard or was not gentle when cleaning my teeth. Ive never had such experience in my entire left.
i was told i could not have any dental done because i had knee replacement and i had to take some kind of meds before they would do anything, there should have been a way to find this out before i had gone in. also even if i could have had a filling done i should NOT have come back for that at a different visit. why cant your people do EVERYTHING on one visit instead of a person having to schedule more appointments. it seems that you schedule separate appointments for everything so that you can get more money!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!!

I came in for a specific procedure. There was really nothing to discuss. However, the dentist kept asking me to let him know if I felt any pain and he made sure I knew that at some point he through it was necessary to cap the tooth that I was having filled
My only complaint is when the lab person came up to speak with the dentist she said when I broke a tooth out previously the wrong acrylic was used and that was why my denture cracked. If that was the case this repair should have been done free of charge,
He was not compassionate of my feelings of having multiple extractions. I was upset and he tried to force me to make a decision that moment to have the extractions done right then. When a patient is losing four teeth at once it's difficult to process

Problem is that specialist do NOT take the time to explain options.
Tried to explain that I was not positive about which tooth was the problem - seemed uninterested in exploring possibilities.

Being a former dental hygienist at Dental Associates, I had greater expectations from the exam than that which I received. You see, at the CCD clinic the providers (dentists and hygienists) were well known for their expertise, thoroughness
He was condescending - didn't "listen" to my view and wants. Didn't thoroughly check my teeth & gums.

The whole examine w/the actual dentist lasted about 5 - 7 minutes.
The situation was odd. I explained that the pain wasn't bad, but he offered me a prescription for pain killers. He gave me a prescription for anti-biotics, but there were no signs of infection and no infection was noted on the dentist's notes.
Bad experience, had throbbing tooth for 2 days made apt. thorough exam, but then I felt as if I were being told I do not have a reason for it to be throbbing. When told to take Motrin stated I had already attempted to control pain with this & other alternatives
