As a service to our patients we will provide an estimate of costs prior to services rendered. All co-payments or other out-of-pocket expenses will be due at time of service. Dental Associates will submit all claims directly to the patient's insurance company. The insurance company will determine the benefit reimbursement. Any remaining balance is the patient's responsibility.

I apologize for not knowing her name but the new Orthodontist for the Main Street Green Bay site is absolutely phenomenal. Very soft spoken but clear on what she is doing and the overall game plan. Cant speak highly enough about her. The previous Ortho I had when I had my braces in High School was straight awful. She is 1000 times better. My hygienists are phenomenal as well.
My experience with the staff has been good
I expressed my concern for a tooth that was longer than the other in the front and my dentist gladly agreed to file down the tooth right after my braces went on. I was overjoyed! She is phenomenal and so was my dental assistant Kirstyn.
When I expressed concern after my brackets came off that teeth had shifted, Dr. Sediqzad put the braces back on & corrected the issue.

the orthodontist didnt even talk to me to explain why my daughters braces need to stay on longer than we were told.
