As a service to our patients we will provide an estimate of costs prior to services rendered. All co-payments or other out-of-pocket expenses will be due at time of service. Dental Associates will submit all claims directly to the patient's insurance company. The insurance company will determine the benefit reimbursement. Any remaining balance is the patient's responsibility.

I have been seeing Dr. Ambrookian at Dental Associates in Wauwatosa for many years and never had a student before. I was not asked if that was okay, I was not warned ahead of time and the result was that I had to hold my mouth open for much longer than normal. I have been diagnosed w/ TMD by Dr. Mackman and it is not OK for me to have my jaw in that position for that long. I had to ask the assistant to stop twice so I could massage each side of my jaw. The appt lasted much longer than it should have and I was very unhappy with my service. Dr. Ambrookian also did not offer to check my temporary crown with my mouth appliance and I have to go back. TMD should be very clear on my chart.
